Acknowledgements I want to thank my advisor, Dr. R. Clint Whaley, for an enormous amount of work on my behalf, and for his excellent academic advice. I want to thank my wife for her support and enthusiasm for my academic pursuits. The following two paragraphs are required text by UTSA: " This Masters Thesis/Recital Document or Doctoral Dissertation was produced in accordance with guidelines which permit the inclusion as part of the Masters The-sis/Recital Document or Doctoral Dissertation the text of an original paper, or papers, submitted for publication. The Masters Thesis/Recital Document or Doctoral Dis-sertation must still conform to all other requirements explained in the Guide for the Preparation of a Masters Thesis/Recital Document or Doctoral Dissertation at The University of Texas at San Antonio. It must include a comprehensive abstract, a full introduction and literature review, and a final overall conclusion. Additional material (procedural and design data as well as descriptions of equipment) must be provided in sufficient detail to allow a clear and precise judgment to be made of the importance and originality of the research reported. It is acceptable for this Masters Thesis/Recital Document or Doctoral Dissertation to include as chapters authentic copies of papers already published, provided these meet type size, margin, and legibility requirements. In such cases, connecting texts, which provide logical bridges between different manuscripts, are mandatory. Where the student is not the sole author of a manuscript, the student is required to make an explicit statement in the introductory material to that manuscript describing the students contribution to the work and acknowledging the contribution of the other author(s). The signatures of the Supervising Committee which precede all other material in the Mas-This dissertation details contributions made by the author to the field of computer science while working to improve the performance of a state-of-the-art linear algebra library intended for use on commodity platforms. These platforms and libraries have become a de facto standard for most scientific researchers. There are three significant contributions to the field. The superblock family of summation algorithms generalizes the dot product and its error analysis, and allows floating point error to be controlled with a desired target bound and performance loss and with workspace determined at compile time. It demonstrates empirically that theoretical error bounds are good predictors of actual errors. The opposite had been asserted by leading experts in the field. The Master Last algorithm identifies and …

Identification and risk assessment of midwife error in the labor using systematic human error reduction and prediction approach
Introduction: Labor is one of the most important wards of hospital, where human error is high. Midwifery errors in the maternity ward and in the delivery can be a serious threat to the health of the mother and the infant, resulting in increased treatment costs. Factors affecting human error are diversity in work, high workload, and fatigue. Therefore, this study aimed to evaluate the midwifery ...
متن کاملA Low Complexity Forward Error Correction for PAPR reduction in OFDM Systems
In this paper, a Peak to Average Power Ratio (PAPR) reduction technique in Orthogonal Frequency Division Multiplexing (OFDM) systems is proposed. This technique uses error correction capability of channel coding to reduce the PAPR.The coded bits at the input of OFDM modulator are separated into two groups of the Most Significant Bits (MSB) and the LeastSignificant Bits (LSB). The bits are mappe...
متن کاملOn the Performance of a Multivariate Control Chart in Multistage Environment
In this paper, a Multivariate-Multistage Quality Control (MVMSQC) procedure is investigated. In this procedure discriminate analysis, linear regression and control chart theory are combined to control the means of correlated characteristics of a process, which involves several serial stages. Furthermore, the quality of the output at each stage depends on the output of the previous stage as well...
متن کاملHuman Error Assessment in City Gate Stations of Isfahan Natural Gas Company Using the System for Predictive Error Analysis and Reduction Framework
Introduction: Human factor analysis has been identified as the most common cause of accidents in natural gas transportation and distribution facilities. The occurrence of accidents at these systems, especially gas reduction stations located in residential and industrial areas, has had catastrophic consequences. Therefore, this study aimed at analyzing critical tasks and human error assessment u...
متن کاملError Management Culture and Its Impact on Hospital Performance: A Case Study in Iran
Background and Objectives: In recent decades medical errors have become a major issue for scientific investigation to avoid potential harmful failures threatening patients’ health and safety. Developing risk management culture has been considered not only to play an important role in detecting and coping effectively with such errors but also lead to high level ...
متن کاملSearch Based Weighted Multi-Bit Flipping Algorithm for High-Performance Low-Complexity Decoding of LDPC Codes
In this paper, two new hybrid algorithms are proposed for decoding Low Density Parity Check (LDPC) codes. Original version of the proposed algorithms named Search Based Weighted Multi Bit Flipping (SWMBF). The main idea of these algorithms is flipping variable multi bits in each iteration, change in which leads to the syndrome vector with least hamming weight. To achieve this, the proposed algo...
